Before you can place an order you need to submit some paperwork so we can verify full compliance with your state law and health code. This may seem like a hassle but this is a one-time setup only and it does not need repeating until your recommendation expires.

Preferably we would like to receive:

Certified/notarized (go to The  UPS Store or any notary public)  or an original copy of;

1. Doctor's note (on his letterhead) with his/her name and practice phone #

2. Include your phone # in mail
